Rare Cases
Scapula fracture:
Scapula fractures have accounted for a significant percentage of fractures lately. Earlier the treatment for these kind of fractures were mostly non surgical which led to deformities in few cases. Usually there are many associated injuries along with the scapula fractures like spine, cranium, and thorax. Generally orthopedician suggests other tests like CT scan for chest, 3D scan of scapula to evaluate the fractures. After thorough evaluation doctor suggests if it can be fixed in a non surgical method or a surgical method.
NON SURGICAL METHOD:
In non surgical method , the shoulder is immobilized from days to weeks depending on the fracture. Then gentle movements or exercises to strengthen the muscle are advised by the doctor.
SURGICAL METHOD:
Depending on the fracture, incisions are made along the scapula. Since scapula is a flat bone less stripping of tissues and muscles is needed for fixation. The complex fracture is reduced and aligned, helping the patient to restore the functionality and get back to normal activities.
Endoprosthetic replacement:
Two decades ago, a tumor in the bone of a limb had no other option but amputation. But with the advances in the medicine, the tumor is removed and bone is reconstructed without affecting the functionality. Endoprosthetic replacement is a typical treatment for tumors around knee and hip which proves low complications and helps the patients to achieve functionality. The bone is reconstructed either with a metal implant or a bone graft. Although chemotherapy can shrink the size of the tumor, it has to be removed.
Stoppa approach for acetabulum fractures:
A fracture to the acetabulum is an intra articular fracture and the procedure for the fracture reduction and fixation is quite complicated as it is a major weight bearing joint in the body. Although there are various other procedures to fix it, modified stoppa approach has good outcome. In this procedure an incision is made above the acetabulum, the nerves and blood vessels under the incision are protected. Once the fracture is viewed it is reduced and internal fixation is done.
The patients are encouraged to do basic exercises as early as possible depending on the severity of the fractures, weight bearing capacity, stability of fixation.
